Admission Process:
An admission process will occur for any and every individual for whom services are being initiated with SESDAC, Inc. The process begins by completing our Application Form and sending or faxing it to the address/fax number found below. The Admissions Coordinator will then review the application and present it to the Admissions Committee for consideration. The committee will review the application and determine if more information or evaluations are needed and have the Coordinator work with the person/family/legally authorized representative to obtain that information.
When it is anticipated that admission may be appropriate, the Admissions Coordinator will begin working with the person, their family and/or legally authorized representatives to schedule a convenient time for the interested parties to meet with staff, tour the agency and discuss the specific needs of the applicant.
Once the agency visit has taken place, the Admissions Coordinator will notify the applicant of the committee’s recommendation. It is possible that the Admissions Committee may determine that further evaluation may be needed prior to making a decision regarding admission of an applicant. Applicants may be placed on a waiting list for admission due to capacity limitations. Although it is rare, an applicant may not be recommended for acceptance.
Once the applicant is accepted, the Admissions Coordinator will schedule a meeting with the person, their family and/or legally authorized representatives in order to complete the final enrollment paperwork.
