A professional position in the human services field specializing in coordinating self-directed services and supports for families having children and/or adults with developmental disabilities living in their family homes.
Requirements: A family member of a person with a developmental disability or a person with a degree in human services, social work, sociology, psychology, or related field with knowledge of existing services; and proven experience and skills in verbal and written communication.
The position requires an individual who has the ability to work independently with minimal supervision once initial training is completed.
Work Requirements: Work cooperatively with the Provider Agency and the Division of Developmental Disabilities to develop and implement a program of supports in the Southeast South Dakota region.
Administer the program by networking with other community resources, developing resources that are not available, and make decisions with families to meet their needs.
Facilitate needs assessments and develop plans of care documenting individual needs and services provided. Complete assessments to assist in determining eligibility for services.
Monitor and document all program expenditures, including administrative costs, service coordination and financial assistance provided to each participant.
Prepare monthly reports outlining key activities; provide reports of utilization of services and funds, participant satisfaction and the impact of service provided as well as other related information.
