Family Support 360

Helping children & adults with disabilities

Family Support 360 is a program dedicated to enhancing the lives of children and adults with disabilities while staying in their homes.

Designed to empower participants and their families, this program places the control firmly in their hands, allowing them to self-direct the essential services required to achieve greater independence within the community.

Every participant in the Family Support 360 Program is partnered with a dedicated Family Support Service Coordinator. This coordinator plays an essential role in helping individuals and their families navigate the available services, both state-funded and community-based. The Department of Social Services will determine the eligibility for this program.
